Please use this form to request an appointment in our clinic.

For Tuberculosis Skin Testing (TST):

We offer TST to those meeting:

• Persons less than 65 years of age prior to admission to a long-term care or retirement home
• Medical surveillance
• Contacts of active TB cases
• Medical screening
• Screening prior to entry into rehabilitation programs

We are not offering TST for:

• Students
• Employment
