I have heard that people have been directed to self-isolate from exposure to a ‘breakthrough case’. What is a ‘breakthrough case?’

A ‘breakthrough case’ means an individual who is fully vaccinated tested positive for COVID-19.

Individuals may be directed by Public Health to self-isolate that have been in contact with this individual regardless of their vaccination status if there is a significantly greater proportion of breakthrough cases than would be expected amongst high-risk close contacts.
