Can I get a record of my immunizations (vaccinations) from Public Health Sudbury & Districts?
Yes. If you reported your immunizations to Public Health Sudbury & Districts, we will have a record on file. We can give you a copy of your records at no cost. If you got immunized at a Public Health Sudbury & Districts’ clinic, we will have a record on file for that immunization.
How to update or access your records

- Please call us if you have questions or would like to speak with a public health nurse.
Important: Health care providers do not automatically report your immunizations to Public Health Sudbury & Districts: it is the patient’s responsibility.
