Where can I get a copy of my COVID-19 vaccination receipt?

Clients are now able to download a digital copy of their COVID-19 vaccination receipts through the provincial Ontario.ca portal. Please visit covid19.ontariohealth.ca and log in using your health card number. For more information, please call 705.522.9200 (toll-free 1.866.522.9200), between 8:30 a.m. and 4:30 p.m., Monday to Friday.
