How to check your immunization records
Your immunization records help you track which vaccines you or your child have received. Keeping your records up to date and in a safe location is important. You may need an immunization record to:
- attend daycare, school, or summer camp
- change schools or if you’re moving to a new area
- apply to college or university, or for job training
- start a new job (some workplaces require vaccines)
- receive medical treatment
- travel to another country
In Ontario, health care providers do not automatically report immunizations to Public Health. You might have different types of records for your family. These can include:
- paper records, like a yellow immunization card or printouts from a health care provider
- electronic records
Children and youth (17 years of age and younger)
To access your child’s records, check with providers where they were immunized, including:
- your health care provider (if your family doctor has retired, contact the Ontario College of Physicians and Surgeons)
- pharmacies or walk-in clinics (they might only keep records for a limited time)
- health care providers or health agencies outside of Ontario or Canada
- schools or daycares (if you submitted the records to them)
Immunization Connect Ontario
You can log into Immunization Connect Ontario (ICON), a secure online Ministry of Health tool, to view any immunizations that were reported to Public Health.
- Immunization records are only recorded in ICON if they have already been sent to Public Health (by email, mail, phone, or directly through ICON).
- Records in ICON may not include all your child’s immunizations.
Public Health maintains records for children and youth 17 years of age and younger as part of its responsibilities under the Immunization of School Pupils Act (ISPA). To update your child’s immunization record, visit our records and reporting page.
Adults (18 years of age and older)
Although Public Health maintains immunization records for children and youth under the ISPA, we might not have complete or updated records for adults.
If you are 18 years of age and older, you can log into ICON to view immunizations that were reported to Public Health if they were:
- received when you were under the age of 18 and reported to Public Health in the past
- reported to Public Health while you attended elementary or secondary school in the Sudbury and Manitoulin districts
- received at a Public Health office or clinic, for example, a community-based flu clinic or school clinic
If you are looking for your records, you can also check:
- your health care provider’s office (if your family doctor has retired, contact the Ontario College of Physicians and Surgeons)
- pharmacies or walk-in clinics (they might only keep records for a limited time)
- health care providers or health agencies outside of Ontario or Canada
- previous employers or schools, colleges, or universities (if you submitted the records to them)
- at home or with family members, as parents or guardians might have copies of your childhood records

COVID-19 vaccine records
If you received any dose of the COVID-19 vaccine in Ontario, your records can be accessed through the provincial COVID-19 vaccine portal (Ontario Government).
