This is a reminder that Public Health Sudbury & Districts’ regular hours of operation are Monday to Friday 8:30 a.m. to 4:30 p.m. For statutory holidays and non-statutory holidays, please visit www.phsd.ca.
Should agencies or highest risk settings require immediate support on weekends, holidays, or after hours (before 8:30 a.m. or after 4:30 p.m.), Public Health’s on-call staff can be reached at 705.688.4366.
- If a highest risk setting suspects that they are in an outbreak, they must contact our on-call staff via 688.4366 as soon as possible AND submit a line list to Line_Listing_Reporting@phsd.ca (preferred) or fax to the confidential fax line, 705.677.9618.
- Facilities that require Public Health consultation for admissions/transfer requests must call our on-call staff via 688.4366 AND submit the transfer and admissions checklist to the confidential fax line, 705.677.9618.
Reminder: If a facility is reporting a Disease of Public Health Significance identified as requiring immediate reporting to Public Health on a weekend, holiday, or after hours, notification to Public Health must be completed by calling our on-call staff via 705.688.4366 AND submitting the Communicable Disease Notification Form to the confidential fax line 705.677.9618.
